(1) A person who wants to register his/her name in the council for doing health business should submit an application to the council in the format as per schedule-1.
(2) When applying in accordance with sub-rule (1) in addition to the educational qualification or degree and certificate of training prescribed in accordance with sub-rule (2) of rule 3, the documents given by the official body in relation to such educational qualification or degree and certificate of training, the name registration fee prescribed in accordance with rule 4 and Nepali In case of a citizen, the certificate of Nepali citizenship and in the case of a non-Nepali citizen, a passport or an identity card issued by a government agency to prove his/her identity must be attached.
